Welcome

We cover both brand name drugs and generic drugs. Generic drugs have the same active-ingredient formula as a brand name drug. Generic drugs usually cost less than brand name drugs and are rated by the Food and Drug Administration (FDA) to be as safe and effective as brand name drugs.

What is a Formulary?

A formulary is a list of covered drugs which represents the prescription therapies believed to be a necessary part of a quality treatment program. We will generally cover the drugs listed in our formulary as long as the drug is medically necessary, the prescription is filled at a network pharmacy, and other plan rules are followed. For more information on how to fill your prescriptions, please review your Evidence of Coverage.

If you have medical circumstances that may warrant an exception to drug-specific benefit or coverage limitations or need access to a drug not on the plan formulary, you or your prescribing physician may file a request for an exception with Magellan Rx Management. Magellan Rx is the Pharmacy Benefit Manager (PBM) for Alliant Health Plans.

  • Search by typing part of the generic (chemical) and brand (trade) names.

    Brand Name Drugs with Brand-Equivalent Generic

    From time to time, we evaluate the formulary for changes that will benefit our members. One such case is with brand-name vs. brand-equivalent generic drugs, which have the same active ingredients. When a brand-equivalent generic drug becomes available on the formulary in place of a brand-name drug, members benefit in most cases by paying the lower generic tier for their medication. In these situations, the brand-name drug will move to a different tier and/or will be excluded from the formulary. If you notice your brand-name drug's tier has changed or is now excluded from the formulary, please confirm that a generic brand-equivalent drug is available.

    Health Care Reform (HCR) Drug Information

    The drugs labeled with HCR in this tool are available with a prescription (including applicable over-the-counter medications) at no member cost share if certain requirements are met.
